This policy covers the Spinal Decompression Salt Lake City website, which is operated for Mecham Chiropractic. It explains what the website itself collects. How the clinic handles information once you become a patient is covered by the clinic's own policies and the privacy notices it gives patients.
What the website collects
The site has no user accounts and doesn't run advertising trackers. It collects information in two ways.
- The scheduling form. The name, email address, phone number and any message you enter are sent to Mecham Chiropractic through a form service provided by HighLevel (LeadConnector), so the clinic can contact you.
- Standard technical data. Like any website, the servers and services that deliver these pages receive your IP address and basic browser information when a page loads. That includes the web host, the photo service Unsplash and the icon library served by jsDelivr.
Please don't send medical records or detailed health information through the form. Bring those to your appointment instead.
How it's used
Form information is used to respond to your request. Technical data is used by the services above to deliver and secure the pages.
